Lower Back Pain

Question:

I have a sore lower back, disc degeneration. Should I be using hot or cold compresses for relief?
- Milligan

Answer:

As a general rule of thumb, the hydrotherapy protocol that an R.M.T. would use would be as follows:

  • Acute Lower Back Pain Injury: first 24-48 hours = Ice to area, no longer then 20 minutes.
  • Sub-Acute Lower Back Pain Injury: 48 hours to potentially 2 weeks = Vascular Flush (alternating hot and cold for 10 minutes each, repeating three times).
  • Chronic Lower Back Pain Injury: 3 weeks on = Heat to area again no longer than 20 minutes.

You should always consult a health care provider to rule out any contraindications to hydrotherapy or even more serious conditions such as fractures, especially when it comes to lower back pain. In relation to the lower back pain question above disc degeneration is usually a chronic condition with occasional flair ups; therefore, heat would most likely be indicated.
